23 <br />111. Results of 2009 Adopted Priorities <br />Objective A-6: (New Initiative for 2009) Begin the three-year Community Resource <br />Connections(CRC) project for improved access to services for older adults and <br />persons with disabilities. <br />Lead Organization(s): Department on Aging's Aging Transitions Div. with County Social <br />Services' Adult Services Unit and County Information Technologies <br />Activities and Accomplishments: 2008-09 <br />1. The Chatham Orange CRC was launched on November 30, 2009, as was a new website: <br />www.chathamorangecrc.org/. <br />2. Staff from Chatham and Orange Aging, Social Services, OPC, UNC Hospitals, NC Access <br />Care, Alliance for Disability Advocates/Center for Independent Living, and numerous consumers <br />representing the interests of the aging and disability communities worked together to cross train <br />staff and develop procedures to make consumer access to information and services person- <br />centered rather than agency-centric. <br />3. The Chatham Orange CRC has been awarded additional funding to participate in a developing <br />person-centered hospital discharge plan. <br />4. Carol Woods' Community Connections grant project held atwo-day consumer input forum in <br />Hillsborough with transitions in care identified as a community priority among older residents. <br />Recommendations for 2010: <br />1. Information Technologies support for CRC confidential communication among departments. <br />2. Establish CRC policies and procedures that enable the state's mandate of "No Wrong Door" a <br />reality in practice. <br />Objective E-6: (New Initiative for 2009) Improve food service at Senior Centers in <br />conjunction with supporting the local economy and environment. <br />Lead Organization(s): Department on Aging-Senior Center Operations Div. with the <br />County Advisory Board on Aging, Friends of the Senior Centers (Seymour & Central) <br />Activities and Accomplishments: 2008-09 <br />1. Senior Lunch Program: Beginning on July 1, 2009, Orange County Department on Aging <br />began administering the Senior Lunch Program, a federally funded meals service to older adults <br />60+ on a donation basis. With the assistance of the Food Services Innovations Committee, the <br />program went "Green", becoming more environmentally friendly by no longer using disposable <br />products (ex. plates, silverware, and cups) as in the past. Both Friends of the Senior Center at <br />Seymour and Central Orange helped by purchasing dishes, cups and plateware and the <br />Department purchased the chemicals for the commercial dishwashing machines, saving an <br />estimated $8000 per year. The program began "Employing Local" by hiring and training Title V <br />Senior Employment workers to work in the kitchens and operate the dishwashing machines. On <br />November 1, 2009, the program began "Buying Local" by contracting with Nantucket Grill, a <br />local restaurant in Chapel Hill, to cater 150 meals per day to the senior centers and adult day <br />21 <br />
