The Health Department has received revenue for the following programs: <br />• Diabetes Self-Management Education Program - $750 in additional funds from the <br />North Carolina Public Health Foundation for support of this program. The goal of the <br />program is to achieve better management of diabetes, increase access through an <br />active treatment and referral network, improve health outcomes, and reduce <br />healthcare costs. <br />
