6 <br />CHECKLIST OF DOCUMENTATION REQUIRED <br />WITH THIS APPLICATION <br />APPLICATION <br />You must provide an ORIGINAL plus ONE Copy of the application, which consists of the <br />following sections: <br />^ Section I: Applicant Information <br />^ Section II: Project Description (including location and site maps) <br />^ Section III: Project Budget and Pro-forma <br />^ Section IV: Agency Description <br />^ Section V: Disclosure of Potential Conflicts of Interest <br />OTHER REQUIRED ATTACHMENTS <br />Please provide one copy of each of the following documents: <br />^ Current list of Board of Directors. Include addresses, phone numbers, terms, and <br />relevant affiliations <br />^ Minutes of Board of Directors meeting authorizing the submittal of this <br />application <br />Please provide one copy of each of the following documents unless they are already on file <br />with the Town or County (please check Town/County staff to ensure documents are on file): <br />^ Current Bylaws and Articles of Incorporation <br />^ IRS tax determination letter (501(C)(3) <br />^ Most recent independent audit (including management letter if issued) <br />DOCUMENTS REQUIRED AFTER AWARD <br />Before signing Performance/Development Agreements with the Town/County, the funded <br />agency must provide the following documents unless they are already on file with the Town or <br />County (please check Town/County staff to ensure documents are on file): <br />^ Personnel policies <br />^ Financial management procedures <br />^ Procurement policy <br />^ Conflict of interest policy <br />V <br />
