2) A signed statement from the applicant certifying that the applicant's <br />proposed Wireless support structure shall be maintained in a <br />safe manner, and in compliance with all conditions of all <br />applicable permits and authorizations, without exception. as well <br />as all applicable and permissible local codes, ordinances, and <br />regulations, including any and all applicable County. State and <br />Federal Laws, rules, and regulations. <br />31 A signed statement from the applicant certifying that the <br />applicant is duly authorized to do business in the State. and if <br />the applicant is a foreign entity, the application must contain a <br />certified copy of the entity's Certificate of Authority from the <br />office of the North Carolina Secretary of State. <br />4)A site plan prepared in accordance with Article MV of this <br />Ordinance showing the footprint, location and dimensions of <br />access drives, landscaping and buffers, fencing as well as all <br />other requirements for site plans required by Article XIV, <br />5) Elevation drawings showing the vertical rendition of the wireless <br />support structure identifying all users, attachments. and all <br />related fixtures, structures, appurtenances and apparatus . <br />including height above the lowest adjacent pre - existing _grade . <br />materials, color and lighting and shielding. <br />6)The azimuth, size and centerline height location of all proposed <br />and existing antennas on the supporting structure. <br />7)The number, type and model of the antenna(s) proposed, along <br />with a copy of the specification sheet(s) for the antennas: <br />8)The age of the existing tower (if any) in years. including the date <br />of the grant of the original permit or authorization for the existing <br />tower: <br />9)A description of the type of wireless support structure. e.a. <br />guyed, self - supporting lattice or monopole: <br />10) The make, model. type and manufacturer of the wireless <br />support structure and the structural design calculations. certified <br />by a Professional Engineer licensed in the State, proving the <br />structure's capability to safely accommodate the facilities of the <br />applicant without change or modification, or if any change or <br />modification of the structure is needed, a detailed narrative <br />changes are made: <br />12) For a wireless support structure that is five (5) years old or <br />older, or for a guyed structure that is three (3) years old or older, <br />a copy of the latest ANSI Report done pursuant to the latest <br />edition of ANSI- EIA/TIA 222F — Annex E for any self - supp orting <br />
