STATE OF NORTH CAROLINA <br />COUNTY OF ORANGE <br />RETURN THIS COPY TO THE CLERK'S <br />'OFFICE FOR THE PERMANENT AGENDA FILE"— <br />AGREEMENT <br />IW-- /a -/J-- �r8 <br />THIS AGREEMENT, made and entered into this first day of January 1999, by and between Chapel Hill <br />Training - Outreach Project, Inc., hereinafter referred to as " CHTOP," for its Early Head Start Program, hereinafter <br />referred to as "EHS" and the Orange County Health Department, hereinafter referred to as the "OCHD." <br />WITNESSETH: <br />WHEREAS, the EHS desires to provide family support services for families enrolled in the Early Head <br />Start Program, and, <br />WHEREAS, the OCHD provides family support services in its intensive home visiting and child service <br />coordination programs. <br />NOW, THEREFORE, in consideration of the premises and the following mutual covenants and conditions <br />and any sums to be paid, the OCHD and CHTOP agree as follows: <br />1. The OCHD agrees to provide two (2) social worker positions for the provision of family support services to <br />EHS participants. The social workers' duties will be to: <br />a. Provide family service coordination to a caseload of 20 -25 children and their families with complex <br />medical and social needs. Provide indirect services to an additional 20 -25 families receiving case <br />management services from other community agencies in order to support the accomplishment of goals <br />identified in preexisting family plans. <br />b. Provide community outreach services in order to identify and recruit potentially eligible families for the <br />Early Head Start Program. <br />c. Facilitate the initial linkage of families to the Early Head Start Program and initiate and assist with the <br />enrollment process. <br />d. Assess each families strengths and needs at enrollment and on an ongoing basis. <br />e. Assess parent/child interaction at periodic face to face family contacts. <br />f. Develop with families a comprehensive service plan and family partnership agreement based on family <br />priorities, interests, strengths, and needs. Reassess and revise plan on a regular basis. <br />g. Develop a supportive, non judgmental relationship with families, respecting individuality and cultural <br />differences. <br />h. Educate families regarding community resources and make appropriate referrals for needed services. <br />i. Provide crisis intervention counseling and support to families in emergency situations. <br />j. Provide transportation services to activities which are part of Early Head Start's mission and family goals <br />when no other transportation resources are available. <br />k. Coordinate services to families provided by a variety of resources and advocate on families behalf in a <br />professional and positive manner in order to help family appropriately access needed services. <br />1. Make regular home visits to families to follow -up on service plan and to provide counseling and parent <br />education. <br />m. Assure that child receives regular health and developmental screenings. <br />
