recovery center <br />MEMORANDUM OF AGREEMENT <br />BETWEEN <br />Freedom House Recovery Center AND Club Insight <br />April t, 2008 <br />This memorandum of agreement is between Freedom House Recovery Center PMC) and <br />Club Insight The purpose of this memorandum is to outline the procedure used for <br />interfacing services for the clients attending Freedom House Recovery Center Resource <br />Center and Chatham Counseling Center (CCC). <br />Parties agree as follows: <br />Club Insi t agrees: <br />1. To maintain communication with the FHRC Resource Cznter <br />as needed for evaluating and improving client services. <br />2. To provide a day program for severe Mental illnesses as a diagnosis. <br />3. To build on rebuilding daily living skills to prepare client to be functional in <br />daily living. <br />4. To provide transportation to and from the facility at no cost to the client. <br />S. To provide these services at no cost to Freedom House Recovery Center or <br />Chatham Counseling Center. <br />FHRC's Coordinator and/or Case Mmager agree: <br />1. To obtain all required release of information foi-ins needed to facilitate the <br />exchange of information between agencies. <br />2. To provide any pertinent information about the client to insure appropriate <br />semces. <br />3. To provide results of Substance Abuse Assessment/Mental Health <br />Assessment of clients. <br />4. To refer clients to Club Insight <br />This memorandum may be terminated by either party or by mutual consent of both parties <br />involved, provided that written notice of such intent is given thirty (30) days prior to the <br />effective date of termination. <br />Kat coie4kecuttve birector Trish Hussey Executive Di ctor <br />
