recovery center <br />MEMORANDUM OF AGREEMENT <br />BETWEEN <br />Freedom House Recovery Center and <br />PTA Thrift Shop Inc. <br />This memorandum of agreement is between Freedom House Recovery venter PMC) and <br />PTA Thrift Shop Inc. This agreement is effective from April 7, 2008 through April 7, 2009. <br />The purpose of this memorandum is to outline the procedure used for interfacing clothing <br />needs for the clients attending Freedom House Recovery venter Resource Center and <br />clients being served in Orange/Person/Chatham counties by the FHRC CSS worker. <br />Parties agree as follows: <br />PTA Thrift Inc. agrees: <br />1. To maintain communication with the FHRC Resource CxrAff <br />CSS worker as needed for evaluating and improving client services. <br />2. To provide clothing apparel for clients with an approved voucher signed by <br />the CSS worker for Freedom House. Apparel will be as follows; <br />• Men: 1 sub 1 dress shirty 1 necktie, I pair of dress dxx s <br />■ women: 1 pewit or dressy 1 pair of dr= shoes <br />3. To provide Ns service and accept reimbursement on a monthly account set <br />up for Freedom House. <br />4. To provide a monthly sWzient ' .. account activity. <br />FHRC's Coordfimtor and/or Case Manager agree: <br />1. To provide t unsporlation to PTA Thrift Inc. and shop with client to acquire <br />ate apparel and have voucher avOkle. <br />2. To provide any pertinent information about the client to insure appropriate <br />. <br />semces. <br />3. To provide this service at no cost to PTA Thrill Shop Inc.. <br />This memorandum may be terminated by either party or by mutual consent of both parties <br />involved, provided that written notice of such intent is given thirty (30) days prior to the <br />effective date of termination. <br />_ 1 <br />Barbara Jess --.
