re cove ry e n to ar <br />MEMORANDUM OF AGREEMENT <br />BETWEEN <br />Freedom House Recovery Center AND <br />Orange County Health Department <br />This memorandum of agreement is between Freedom House Recovery Center THRC) and <br />Orange County Health Department. This agreement is efEecttve as of Aptil 11 2008. The <br />purpose of this memorandum is to outline the procedure used for interfacing services for <br />the clients attending Freedom House Recovery Center Resource Center. <br />Parties agree as follows: <br />Orange County Health Department agrees : ' <br />1. To na.aintain communication with the FHRC Resource Center <br />Coordinator as needed for evaluating and improving client services. <br />2. To provide appropriate health education, clinical, and HIV counseling and <br />testing services to address client needs. <br />3. To provide billable services on a sliding fee scale or billed to Medicaid, and other <br />services at no charge to FHRC or the client. <br />4. To provide HIV Case Management and Education to clients if appropriate. <br />FHRC's Resource Center Staff agree: <br />1. To obtain all required release of information forms needed to facilitate the <br />exchange of information between agencies. <br />2. To provide any pertinent information about the client to insure appropriate <br />services, . <br />3. To provide meeting space for tutors and clients. <br />4.
