recovery center <br />MEMORANDUM OF AGREEMENT <br />BETWEEN <br />Freedom House Recovery Center AND <br />Vocational Rehabilitation <br />This memorandum of agreement is between Freedom House Recovery Center <br />' Rehabilitation (VR). This a eement is effective -as of April <br />�FHR�} and �iTocatsonal �' <br />1 2008. The purp ose of this memorandum is to outline the procedure used for <br />interfacing services for the clients attending Freedom House ' Recovery Center <br />Resource Center. <br />Parties agree as follows <br />VR agrees: <br />1. To maintain communication with the FHRC Resource Center <br />Coordinator as needed for evaluating and improving client services. <br />2. To provide appropriate evaluation, Wsting and/or counseling to appropriately <br />meet the client service needs for j ob skills training, employment, and housing. <br />3. To provide this service at no charge to FHR.0 or the client. <br />FBRC's Coordinator and/or Case Manager agree: <br />1. To obtain all required release of information forms needed to facilitate the <br />exchange of information between agencies. <br />2. To provide any pertinent information about the client to insure appropriate <br />services. <br />3. To p rovide results of Substance Abuse Assessment Instrument s), if <br />applicable. <br />4.
