Rp r. 1 I, 1UUd J: U5VM � reedom House Kecove ry <br />OIL � <br />recovery center <br />MEMORANDUM OF AGREEMENT <br />BETWEEN <br />Freedom House Recovery Center AND <br />Orange Chatham County Probation <br />April I, 2008 <br />No. 1911 F. 5 <br />This memorandum of agreement is between Freedom House Recovery Center (FHRC) and <br />Orange- Chatham County Probation and Parole Services. The purpose of this memorandum <br />is to outline the procedure used for interfacing services for the clients attending Freedom <br />Rouse Recovery Center Resource Center. <br />Parties agree as follows: <br />1. Probation will have clients with substance abuse problems report to TASC for an <br />evaluation/assessment. TASC will provide Probation with a written report and <br />recommendations. <br />2. When the Probation Officer refers an offender to TASC, the following documents will be <br />sent to TASC: Probation judgment, any orders modifying Probation, Criminal Records <br />Check and positive drug screens. TASC will assure that these records are then shared with <br />the Resource Center, <br />3, Individuals assigned to the Resource Center will be given IOP T or RAPT, Education <br />Training,! Life Skills community support, -computer skills tralnm =i vocational education <br />support and CBI classes. <br />4. Probation will maintain contact with clients at the Resource Center and elsewhere and will <br />enforce the clients obligations to attend. <br />S. Drug testing will be performed by the Supervision Probation Officer. The Resource Center <br />will perform a multiple dip test within 30 days of a client's entrance into the Resource Center, <br />6. Probation will provide failure -to- attend follow up of clients. <br />7. The Resource Center will provide Probation with verbal and telephone reports at any time, <br />At the end of each month, the Resource Center will provide Probation with a complete <br />written report on the individual covering the month's activities. <br />S. Upon client failure to satisfactorily participate, the Resource Center will P rovide Probation <br />with a written report documenting that in order for action to be taken. <br />9.
