• <br />Social Services — Existing Social Worker I <br />Request: Increase the hours of a permanent part time (20 hours) Social Worker I to <br />permanent full time (40 hours). <br />Purpose: To increase the support provided to the Community Alternatives program for <br />Disabled Adults (CAP/DA) to the end that adults who qualify for skilled nursing care may <br />remain at home and out of a nursing home facility. <br />Job Functions: <br />❑ Conducts assessment of physical needs, family support and mental status of disabled <br />adults. Develops and revises plan of care. <br />❑ Makes home visits to provide ongoing assessment of needs and provision of services. <br />Arranges for services such as medical supplies, durable medical equipment, in -home aide <br />services and the like. Monitors delivery of services such as aide time, supplies, nursing <br />visits and the like. <br />❑ Prepares documentation to meet State and Federal regulations. <br />Reason for Request: <br />To ensure that staffing hours are available to provide case management support to individuals <br />who qualify for the Community Alternatives program. In the past, a waiting list of <br />individuals has been formed due to lack of staffing hours to support the program. <br />Outcomes: <br />❑ Enable 25 disabled adults to receive comprehensive medical and social assessments and <br />plans of care. <br />❑ Prevent 15 nursing home placements and save over $1,000 per placement in federal, state <br />and local funds. <br />❑ Enhance the quality of life for disabled adults by assisting them to remain in their homes <br />and prevent elder abuse and neglect. <br />Financial Impact: <br />Additional Position Cost <br />(Salary/Benefits) <br />Offsetting <br />Revenue/Budget <br />Net Additional 99 -00 <br />Position Cost <br />$14,974 <br />$14,974 <br />$0 <br />Social Services advises the cost of the additional hours will be offset by Medicaid revenue. <br />Relation to Board Goal(s): <br />Not related to a specific Board goal.
