Contractor's Financial Reporting Year July 1, 2007 through June 30, 2008 <br />B. Explanation of Services to be provided and to whom (include SIS Service Code): <br />The Contractor willprovide case management (SIS code 522), information and referral <br />services (SIS code 520) and education and training (SIS code 527) to teen parents in the <br />Orange County Schools System. The Contractor is required to maintain a caseload of 12- <br />16teen parents develop a case plan with each teen outlining specific goals, participate in <br />bi-monthly Community Advisory Committee (CACI meetings, submit a status report by <br />the fifth workday of each month to the County recruit, train, and match volunteers with <br />teen parents and maintain data on the number of teen pazents within the school system. <br />The Contractor is required to meet all goals and outcomes listed in Attachment N. <br />C. Rate per unit of Service (define the unit): <br />1. If Standazd Fixed Rate, Maximum Allowable, (See Rates for Services Chart) <br />2. Negotiated County Rate. <br />$50 000 <br />D. Number of units to be provided: 1 Full-time staff person for aten-month school <br />year including benefits training and travel costs. <br />E. Details of Billing process and Time Frames; The County will reimburse the Contractor <br />• for services described in this contract ~ to the budgetary limits of the contract allotment, <br />which includes salary benefits travel other miscellaneous expenses, indirect costs and <br />Contract-Scope of Work (06/04) Page lof 2 <br />
