Explanation of Services to be provided and to whom (include SIS Service Code): <br />The Contractor will provide case management (SIS code 522), information and referral <br />services (SIS code 520) and education and training (SIS code 527) to teen parents in the <br />Chapel Hill-Carrboro City Schools System. The Contractor is reauired to maintain a <br />caseload of 12-16 teen parents develop a case plan with each teen outlining specific <br />goals partic~ate in bi-monthly Community Advisory Committee (CAC) meetings, <br />submit a status report by the fifth workday of each month to the County, recruit, train, <br />and match volunteers with teen parents and maintain data on the number of teen parents <br />within the school system The Contractor is required to meet all goals and outcomes <br />listed in Attachment N. <br />C. Rate per unit of Service (define the unit): <br />1. If Standard Fixed Rate, Maximum Allowable, (See Rates for Services Chart) <br />2. Negotiated County Rate. <br />$50 000 <br />D. Number of units to be provided: 1 Full-time staff person for aten-month school <br />year including benefits training, and travel costs. <br />E. Details of Billing process and Time Frames; The County will reimburse the Contractor <br />• for services described in this contract up to the bud~etar~limits of the contract allotment, <br />which includes salary benefits travel other miscellaneous expenses indirect costs and <br />Contract-Scope of Work (06/04) Page lof 2 <br />
