Explanation of Services to be provided and to whom (include SIS Service Code): <br />The Contractor will provide case management (SIS code 522), information and referral <br />services (SIS code 520) and education and training (SIS code 527) to teen parents in the <br />Chapel Hill-Carrboro City Schools System. The Contractor is reauired to maintain a <br />caseload of 12-16 teen parents develop a case plan with each teen outlining specific <br />goals partic~ate in bi-monthly Community Advisory Committee (CAC) meetings, <br />submit a status report by the fifth workday of each month to the County, recruit, train, <br />and match volunteers with teen parents and maintain data on the number of teen parents <br />within the school system The Contractor is required to meet all goals and outcomes <br />listed in Attachment N. <br />C.
