Explanation of Services to be provided and to whom (include SIS Service Code): <br />The Contractor will provide case management (SIS code 522), information and referral <br />services (SIS code 520 and education and training (SIS code 527 to teen parents in the <br />Chapel Hill-Carrboro Citv Schools System. The Contractor is required to maintain a <br />caseload of 12-16 teen parents, develop a case plan with each teen outlining specific <br />goals, participate in bi-monthly Community Advisory Committee (CAC) meetings, <br />submit a status report by the fifth workday of each month to the County, recruit, train, <br />and match volunteers with teen parents, and maintain data on the number of teen parents <br />within the school system. The Contractor is required to meet all goals and outcomes <br />listed in Attachment N. <br />C. Rate per unit of Service (define the unit): <br />1. If Standard Fixed Rate, Maximum Allowable, (See Rates for Services Chart) <br />2. Negotiated County Rate. <br />$50,000 <br />D. Number of units to be provided: 1 Full-time staff person for aten-month school <br />year, including benefits, training and travel costs. <br />E.
