Primary medical supervision of the nurse practitioners who are employees of <br />QCH D to i ncl ude ongoi ng monitoring and evaluation of the reed icai acts <br />erforned b the nurse practitioners; Regular individual meetings with .the nurse <br />p Y <br />practitioners will occur according to an agreed upon schedule to review records <br />of high-risk or problematic patients; Documentation will be recorded in the <br />patient record indicating that a review occurred and noting any recommended <br />changes i n the pl an of care. <br />c. Co-signing charts of patients seen by new nurse practitioners of CJCHD within <br />five X57 working days for at least the first six months of their nurse practitioner <br />ractice, and face-to-face consultation on a weekly basis for the first month of <br />p <br />their nurse practitioner practice and at least monthly thereafter for the <br />succeeding five months; <br />d. A review of all C~CHD prenatal records at 1 ~, 28 and ~~ weeks to assure that <br />appropriate medical care is being provided, with a Maternity Chart Review fora <br />completed at each interval. <br />e. Illleekly group meetings with clinical staff in each site according to an agreed <br />upon schedule to discuss patient care and clinical operation issues. <br />f. Qverall responsibility far medical Residents and medical students working in <br />clinics including orientation, supervision of practice and assurance of <br />compliance with QCHD program and documentation requirements. <br />g. provision of routine medical services during scheduled on-site times at C~CHD <br />Qutpatient Clinics, including documentation of Clinical Services provided per <br />established QCHD protocols; <br />h.
