Explanation of Services to be provided and to whom (include SIS Service Code): _ <br />The Contractor wil~rovide 5 FTE qualified staff person to provide access to mental <br />health services to children and their families who are clients of the County, and <br />assure that children and their families served by the Department have access to all needed <br />services including_,early identification and time-critical intervention. The Contractor is <br />required to meet all goals and outcomes listed in Attachment N. <br />C. Rate per unit of Service (define the unit): <br />1.
