Explanation of Services to be provided and to whom (include SIS Service Code): _ <br />The contractor will facilitate early identification and treatment of children who are at risk <br />for Mental Health (social emotional problems This will include establishing routine <br />mental health screening for children age birth through 5 who are receiving Child <br />Protective Services and providin family-focused mental health assessment for those <br />children identified by screening. The Contractor is required to meet all goals and <br />outcomes listed in Attachment N. <br />C. Rate per unit of Service (define the unit): <br />1. If Standard Fixed Rate, Maximum Allowable, (See Rates for Services Chart) <br />2. Negotiated County Rate. <br />$60/hour <br />D. Number of units to be provided: <br />E.
