~g'~~~ <br />recovery center <br />MEMORANDUM OF AGREEMENT <br />BETWEEN <br />Freedom House Recovery Center AND Chatham Transit <br />April 1, 2008 <br />This memorandum of agc+eement is between Freedom House Recovery Center (FHRC) and <br />Chatham Transit The purpose of this memorandum is to outline the procedure used for <br />interfacing services for the clients attending Freedom House Recovery Center Resource <br />Center and Chatham Counseling Center (CCC). <br />Parties agree as follows: <br />Ctta~atn Transit agrees: <br />1. To maintain communication wide the FHRC Resounce Center <br />Coordinator as needed far evaluating and improving client services. <br />2. To provide transportation for clierrts in the Chatham County area to agency <br />related services. <br />3. To allow clients with Medicaid to have access to tKansportation and then bill <br />DSS. <br />4. To provide tiransportation to and from the facility at no cost to the client. <br />FHRC's Coordinator and/or Case Manager agree: <br />1. To obtain all required release of infom~ation forms needed to facilidate the <br />exchange of information between agencies. <br />2. To provide any pextine~t information about the client to insure appropriate <br />services. <br />3.
