~G.~?.P.~Qc~/ltf. ~ <br />recovery center <br />MEMORANDUM OF AGREEMENT <br />BETWEEN <br />Freedom House AND JOCCA <br />April 1, 2008 <br />This memorandum of agreement is between Freedom House Recovery Center (FHRC) and <br />Joint Orange-Chatham Community Action Inc. (JOCCA). The purpose of this <br />memorandum is to outline the procedure used for interfacing services for the clients <br />attending Freedom House. Recovery Center Resource Center and Chatham Counseling <br />Center (CCC). <br />Parties agree as follows: <br />JOCCA agrees: <br />1. To maintain communication wide the FHRC Ressouree Center <br />Coordinator as needed for evaluating and improving client services. <br />2. To provide client usage of corrr~rters to design ms's; employment <br />search and file unemployment claims <br />3. Clients will have access to CSBJ (Community Services Block Program) <br />which is a self sufficient program to aid clients in gelling jobs, to reach <br />education goals and aneagency assistance. <br />4. To provide this service at no charge to FFiRGCCC, or the client. <br />FHRCs Coordinator and/or CSS worker agree: <br />1. To obtain all required release of infaanation fates needed to facilitate the <br />exchange of information between agencies. <br />2. To provide any pea~tinent information about the client to insure aplxopriate <br />services. <br />3. To refer clients to JOCCA. <br />This memorandum may be terminated by either party or by mutual consent of both parties <br />involved, provided that written notice of such intent is given thirty (30) days prior to the <br />effective date of termination. <br />Trish Hussey, cutiv hector <br />Freedom House Recovery Center <br />
