~%E5`i~p~J.G /r~.e f.~ <br />recovery center <br />MEMORANDUM OF AGREEMENT <br />BETWEEN <br />Freedom House Recovery Center AND Club Insight <br />April 1, 2008 <br />This memorandum of agreemem is between Freedom House Recovery Center (FHItC} and <br />Club Insight. The purpose of this memorandum is to outline the procedure used for <br />interfacing services for the clients attending Freedom House Reoovery Center Resource <br />Center and Chatham Counseling Center (CCC). <br />Parties agree as follows: <br />Club agrees: <br />1. To maintain communication with the FHRC Resoucre Center <br />Coordinator as needed for evaluating and improving client services. <br />2. To provide a day program for severe Mental illnesses as a diagnosis. <br />3. To build on rebuilding daily living skills to prepat+e client to be functional in <br />daily living. <br />4. To provide transportation to and from the facility at no cyst to the client. ' <br />5. To provide these services at no cost to Freedom House Recovery Center or <br />Chatham Counseling Center. <br />FHRCs Coordinator and/or Case Manager agree: <br />1. To obtain all required release of information fomos Headed to facilitate the <br />ex~ange of information between agencies. <br />2. To provide any pertinent information about the clieht to insure appropriate <br />services. <br />3. To provide results of Substance Abuse Assessment/Mental Health <br />Assessment of clients. <br />4.
