recovery center <br />MEMORANDUM OF AGREEMENT <br />BETWEEN <br />Freedom House Recovery Center AND <br />Orange County Literacy Council <br />This memorandum of agreement is between Freedom House Recovery Center (FHRC) and <br />the Orange County Literary Councr7. This agteetneat is effective as of APB 1, 2008. The <br />purpose of this memorandum is do outline the procedure used for interfacing services for <br />the clients attending Freedom House Recovery Center Resource Center. <br />Patties agree as follows: <br />Literacy Council agrees: <br />1. To maintain communication with the FHRC Coordinator as needed for evaluating <br />and improving client services. <br />2. To provide appropriate evaluatioq testing and/or counseling to appropriately meet the <br />client service needs. <br />3. To provide this service at no charge to FHRC or the client. <br />FHRC's Resoutre Center Staff agree: <br />1. To obtain all required release of information farms needed to facilitate the exchange of <br />information between agencies. <br />2. To provide any pertinent information about the client to insure appropriate services. <br />3. To provide meeting space for tutors and clients. <br />4. To provide students with sufficient time to complete a minimum of 12 hours of <br />instruction. <br />5. To provide this service at no cost to Orange County Literacy Council. <br />This memorandum may be terminated by either party or by mutual consent of both parties involved, <br />provided that written notice of such itrtent is given thirty (30) days prior to the effective date of <br />termination. <br />Alice Denson, Executive Director <br />Orange County Literacy Council <br />Trish Hussey, Executive Director <br />Freedom House Recovery <br />
