(_
3.6 NCDENR -Division of Water Quality: Surface Water Protection (SWP)
^ SWP performed a site visit on March 25, 2009, and noted a wetlands complex in parcel.
The proposed access road may cross this complex. A Permit would be required for the
road crossing; either a General Isolated Wetlands permit or a Genera1401 Water Quality
permit, depending on if the U.S. Army Corps of Engineers ([JSACE) determines that the
wetlands are jurisdictional or isolated.
^ Commented that the National Wetlands Index map is not a reliable source to make a
determination of the presence or absence of wetlands.
3.7 NCDENR -Division of Environmental Health
^ Depending on the number of employees, the water well maybe considered anon-transient,
non-community public water system and would require Public Water Supply Section
(PWSS) permitting and approval.

4 ~ ~ Apri116, 2009
