Senior Citizen Health Promotion Program <br /> Grant Project Ordinance <br /> Project Number 11-671 <br /> Be it ordained by the Orange County Board of County Commissioners that pursuant to Section 13.2 of Chapter 159 <br /> of the General Statutes of North Carolina,the following grant project is hereby adopted. <br /> Section 1. This project authorized is the Senior Citizen Health Promotion Program to develop a comprehensive and <br /> coordinated County health promotion and disease prevention program for citizens involving a <br /> partnership of public agencies,University Health Affairs and private groups. <br /> Section 2. The officers of the County are hereby directed to proceed with the grant project in accordance with the <br /> mutual cooperation agreement between the County and the University of North Carolina at Chapel <br /> Hill, as approved by the Board of County Commissioners on June 30, 1997. <br /> Section 3. The following revenue is anticipated to be available to complete this project <br /> Grant Funds (1997-98) $52,434 <br /> Fees for Service(1997-98) $14,835 <br /> Grant Funds (1998-99) $57,645 <br /> Fees for Service (1998-99) $12,000 <br /> Grant Funds (1999-00) $69,250 <br /> Total Revenue $309,980 <br /> Section 4. The following amount is appropriated for this project: <br /> Human Services-Senior Citizen Health Promotion $309,980 <br /> Section 5. The finance officer is hereby directed to maintain within the Grant Project sufficient specific detailed <br /> accounting records to provide the accounting to the grantor agency. <br /> Section 6. Funds may be advanced from the General Fund for the purpose of making payments due. <br /> Reimbursement requests should be made to the grantor agency in an orderly and timely manner. <br /> Section 7. Copies of this grant project ordinance shall be made available to the finance officer for direction in <br /> carrying out this project. <br /> Section 8. Positions authorized through this grant project ordinance include: <br /> 1.0 Full Time Equivalent Senior Health Public Educator position associated with this grant project <br /> This position is authorized through June 30, 2000. Subsequent funding in future years is contingent <br /> upon the availability of grant funds. <br /> Section 9. This ordinance supersedes all previous "Senior Citizen Health Promotion" Grant Project Ordinances. <br /> Section 10.This project, originally adopted May 2, 1994 and subsequently amended, is in effect through June 30, <br /> 2000. <br /> Adopted this 21"day of June 1998. <br />
