~~/~~ <br />July 1, 1999 - A CONTRACT BETWEEN ~~~ <br />June 30, 2000 THE UNIVERSITY OF NORTH CAROLINA AT CHAPEL HILL <br />FOR ITS SCHOOL OF PHARMACY AND <br />THE ORANGE COUNTY HEALTH DEPARTMENT <br />This contract is between the University of North Carolina at Chapel Hill for its School of Pharmacy, herein referred to as the <br />School, and the Orange County Health Department, herein referred to as the Health Department. It is understood that the <br />Division of Pharmacotherapy of the School will direct and provide consultant pharmacy services to the Health Department. <br />It is further understood that there will be a mutual cooperation between the Chair, Division of Pharmacotherapy, School of <br />Pharmacy, and the Director of the Health Department in conducting the activities as described below: <br />I. The Health Department agrees: <br />to allow pharmacy students under supervision to obtain part of their educational experience in the Health <br />Department; <br />2. to provide guidance and assistance with related administrative or technical aspects of providing consultant <br />pharmacy services as requested; <br />to participate in the ongoing evaluation of the role of the pharmacist in the local Health Department; <br />4. to reimburse the School of Pharmacy $416.66 per month ($5,000 per year). The School will submit a bill to <br />the Health Department twice annually, November and May, for consultant pharmacy services provided. <br />to enable the pharmacist to perform all activities mandated by all applicable laws, regulations, and <br />standards; <br />6. to register the pharmacy services with appropriate state and federal.. agencies with the assistance of the - <br />consultant pharmacist; <br />to allow the pharmacist to participate in other related activities in the Health Department if agreed to by both <br />parties; and <br />8. to comply with such non-discrimination laws in its educational:.and/or employment practices as may be <br />applicable in the performance of this contract. <br />II. The University agrees that the School of Pharmacy wi1L• > _ ., <br />1. provide pharmacist staffing sufficient to deliver all of the activities identified in Appendix A: the- staffing <br />pattern to be following will be agreed to by the Chair, Division of Pharmacotherapy, and the Director of the <br />Health Department; the activities of all School personnel within the Health Department shall be the <br />responsibility of the School; <br />2. be responsible for the negligence of its employees and agents to the extent of the North Carolina Tort <br />Claims Act. Further, the University, in consultation as to amount with the OCHD, will provide adequate <br />professional liability insurance for the University's students and personnel, who provide the services <br />described in the agreement. The insurance limits will be stated in a Certificate of Insurance submitted at the <br />time the contract is commenced; <br />assure that pharmacy services meet all applicable laws, regulations and standards; <br />a sharon /dennis/OrgCont.doc <br />page 1 <br />
