4
Orange County Partnership for Young Children for Young Children
Project Abstract
Project Title: The Orange County Community Health Awareness Project
Purpose of the Project: This initiative will strnre to improve health awareness and health behaviors of Orange
County families of young children by promoting new and existing health education/promotion activities and
gents and attempting to link identified families to services in the community. Strategies will be used to target
low-income families and the "historically hard-to-reach" by collaborating with key community agencies. The
Health Awareness Project will fill a critical link between the outreach and health promotion services available in
Orange County and the population of families with young children who have not benefited from the center-based
screenings offered in the past or traditional well child programs. Although the health and safety of young
children encompasses all the factors that impact their growth and developmeM,~this project's activities will focus
upon promoting families' knowledge of and access to well-child care, age-appropriate immunizations and
nutrition, child safety resources, health insurance options and age-appropriate screenings for nutrition, vision,
hearing, dental health, and social and developmental milestones. Helping parents of preschool children to
improve their capability io provide age-appropriate health services will promote childrens' health and help them
to arrive at school healthy and ready to loam.
Population to be Served: There are two primary target populations, Orange County families with preschool-
age children and the agencies that provide health and safety-related services. For several years OCPYC's Child
Health Review Committee has recognized that health promotion services are often more available to children
wt~o are attending registered or licensed childcare settings. Typically, young children of Spanish-speaking
families do not have access to these services due to language barriers and family preferences for familial or
informal caregivers. Furthermore, other young children who are at home with parents, relatives or unlicensed
caregivers may have limited access to community services and agencies that provide health. screenings (vision,
hearing, dental) immunizations, well-child care and developmental assessments. Attempts to provide screenings
and parent education events to families outside-of childcare settings have historically been poorly attended.
The specific Partnership goals that are addressed by this project are: To enhance family education and support;
To enhance early intervention services for children who are at-risk for emotional problems, developmental
delays or disabilities, and to ensure the physical and mental health of young children. The following outcomes
will be achieved by this project: Families are knowledgeable about child development and express confidence in
their parenting skills; Families are knowledgeable and have access to community resources; Children with
special needs are identified prior to reaching kindergarten; Preschool-aged children are properly immunized;
Preschool-aged children have access to consistent health care and have a "medical Home"; and Health
providers collaborate to improve early identification of preschool children with health/mental health needs.
Timetable for this project: be!
Staff: # FTE staff: 1
# Part time staff: 0
# Volunteers: ,~.Q
Total Smart Start funds requested:
Total matching funds provided:
Agency lnfonnation:
Legal Name of Agency: ~
Type of Agency: () Prival
Federal ID #: 56-6000327
Application Contact: RebE
Address: P.O.8181 Hiiisbc
Telephone: 1919) 732-818;
Nine of Agency Diredor:_
finning date: 1 / 9 ending date: X30/00
Position titles: Public Relations) Health Educator
# Volunteer Hours: 40 per month
X47.887 Total Project Cost: X57.515
Cash: ~ Q Inldnd: $9,628
ange Coun Health Deaartment
:e, nonprofit 501(c)3 ()For Profit (X) Public
~i~h Hermann Title: CSC Program Coordinator
Trough.
