z <br />The outreach proposed will encompass many facets and should help achieve high rates of Health <br />Choice re-enrollment, high rates of age-appropriate immunizations, and increase the numbers of <br />children accessing needed services. Anticipated outputs include: <br />^ Reach at least 100 families with preschoolers through interagency participation in one <br />community-wide event in year one and increase to four events in year two. <br />^ Reach at least 900 children through one mass canvass/outreach event such as the Hope for <br />Kids' Annual Big Event. <br />^ Hold at least one comprehensive health promotion screening event for preschoolers in year <br />one and at least three during year two through collaboration with community agencies. <br />^ Create mechanisms to record and measure changes in referrals and utilization of services <br />based upon project efforts with agencies involved. <br />The Community Health Awareness Project proposes funding a position, entitled Bilingual <br />Community Awareness Coordinator, who will develop and spearhead an effective community <br />health awareness and health education campaign targeted to young families, including the Latino <br />population. It is proposed that this be created as a health educator position. The health department <br />is requesting the establishment of atime-limited position for the period 10/1/99 through 6/30/00 <br />with renewal of the position subject to continuation funding from the Partnership an evaluation of <br />outcomes. At this time it is anticipated that the position will report to the Child Service Coordinator <br />in the Personal Health Division of the Health Department. <br />The Orange County Partnership for Young Children Board approved the award of this grant for one <br />year and expressed strong support for a second year of funding. <br />FINANCIAL IMPACT: The total project costs are $57,515 of which the Partnership will fund <br />$47,887 for one year. The remaining $9,628 are in-kind contributions of space and office <br />equipment, supervisory time, and a small amount of office supplies and materials. <br />RECOMMENDATION(S): The Manager recommends that the Board approve the project and <br />establish a new permanent, time-limited health educator position for the period October 1, 1999 <br />through June 30, 2000 to carry out the project. Position renewal will be subject to continuation <br />funding and the evaluation of outcomes as established through the annual Smart Start evaluation <br />plan. <br />
