<br /> Tobacco Trust Fund Commission,1080 Mail Service Center, Raleigh, North Carolina
<br /> 27699-1080. The application may be printed by hand in ink,by typewriter or produced by word
<br /> processor, in 11 or 12 pt.font, on eight-and-half inch by eleven-inch paper. Additional materials
<br /> that will assist the Commission in its deliberation on this project may be attached. The left hand
<br /> margin of any attached page must be at least one inch. All applications, attachments to
<br /> applications and written materials received by the Commission are public records,unless
<br /> determined otherwise by court order or other applicable law. Please do not submit social
<br /> security numbers,tax information or other personal identifying information. If at all possible,
<br /> include an electronic copy(diskette,email)of this proposal with the paper copies. Refer to the
<br /> Commission's Rules,02 NCAC 57 .0101 et seq.,which can be viewed on our web site,
