B. WATER QUALITY PROBLEM <br />12 <br /> Process Steps Response Time Guidelines <br /> <br /> i. Environmental Health Division (OCHD) receives request from <br /> <br />resident directl or throu h another a enc . N/A <br /> ii. OCHD contacts American Stone and advises them of 'the <br /> <br />request and the nature of the request. OCHD staff collects water samples for analysis at the State <br /> Laboratory of Public Health. Confirm a well failure exists; or failure claim within 5 business <br /> b. Deny the well failure claim; or days of receipt of the final sample <br /> c. Recommend further study of the well results.
